Since 2009 I have been involved in reviews whose dominant focus is on what went wrong. We might note strong practice [usually in a discrete paragraph at the end] but rarely do we prioritise actively learning from it. I wanted to change it but for many years it felt like I was failing. Were those around me programmed to maintain a deficit focus?
