Blackpool Safeguarding Adults Board

 

The Blackpool Safeguarding Adult Board is seeking to identify an independent reviewer to complete a newly commissioned Safeguarding Adult Review (SAR).

 

Case Overview

The review relates to a 40-year-old man who was brought to the ED under Section 136. He stayed for about 20 minutes and left without notification within 30 minutes. He was triaged within five minutes as Priority 3 (Suicidal Ideation), with plans for Audit C, COWS, MH trace, and MHLT referral. He was already assessed in the community as needing a Section 2 bed, but none were available.

The Mental Health Liaison Team (LSCFT) saw him in the mother and baby room due to capacity issues. He appeared guarded, worried about being sectioned, and did not fully understand his rights. He declined further engagement, requested a psychiatrist and a cigarette, and went outside with two police officers. He then absconded by running off and jumping a hedge. Police pursued him, but he did not return. Later that afternoon, NWAS reported he had been found deceased and the coroner was notified.

An ongoing review is examining bed availability, the full timeline (community assessment, Section 2 recommendation, Section 136 detention), and risk-assessment processes between services. GP records show a longstanding history of mental health issues, substance misuse, suicidal ideation, and recent worsening paranoia. The GP last saw him on in August 2025, LSCFT IR Service saw him on 10/08/25 for psychosis symptoms linked to substance use.
