Halton Adult Safeguarding Board is seeking to identify an independent reviewer to complete a newly commissioned Safeguarding Adult Review.

The case:
The review relates to a MSR was a 35-year old man who lived in Runcorn with his mother. Following a random attack by a stranger, MSR lived as a recluse in his bedroom at the family home for a period of 11 years. MSR’s mother provided him with alcohol and food via an opening in his bedroom door but had not seen her son for a period of months. MSR would communicate with his mother via mobile phone and could be abusive towards her. MSR was agoraphobic and alcohol dependent. MSR’s mother could hear him moving around his room but did not have sight of him. The night before he died MSR’s mother could hear him vomiting. She phoned the paramedics the following day whereupon they found MSR deceased in his bedroom. The conditions of the room were so poor that the local authority, North West Ambulance Service and police refused to enter.

MSR was open to mental health and the GP. It was felt that MSR’s mother has been a victim too, as she did not get the help and support she needed and was possibly keeping the situation secret from others. MSR’s mother did attempt to access support from MSR’s GP but they advised that due to his alcohol dependency they could not help until MSR presented himself for support. The GP did advise MSR’s mother to continue providing him with alcohol for his dependency as without it he could have seizures. The only engagement from MSR himself was with the GP who had prescribe him Diazepam (it is not known whether MSR took the prescribed medication). MSR’s mother did attend the Women’s Centre on one occasion where she asked for help. She was not in receipt of carer’s support. It was noted that the IOPCC have reviewed the case but there is no further police investigation.
