Richmond and Wandsworth Safeguarding Adult Board
The person was, at the time of her death, a 66-year-old female of Black British Caribbean ethnicity. She died in hospital on 5th January 2025 from influenza. She suffered from anxiety and agoraphobia and had refused home access since 2017, resulting in no face-to-face reviews. Concerns about severe clutter and hoarding were raised in 2014 and 2017. She was previously detained under Section 3 for a psychotic episode and was under Section 117 aftercare, yet multi-agency working and risk assessment were lacking. All Care Act and GP reviews were conducted over the phone.
A Safeguarding Adults Review (SAR) was initiated due to concerns that multi-agency engagement was insufficient, given her health and isolation. Her niece called an ambulance on Boxing Day, leading to her hospital admission. Questions arose about the responsibilities of the care agency, capacity assessments, and the adequacy of safeguarding measures.
We will have to confirm with the reviewer at the point of commissioning what the precise remuneration levels will be for this review.
