The Wiltshire Safeguarding Vulnerable People Partnership is looking for an Independent Author to lead a Safeguarding Adults Review (SAR) to understand how different agencies worked together in the months preceding the death of Wiltshire resident, Adult P.  The rationale for this referral is:

• High levels of neglect and self neglect
• Capacity of Adult P to make decisions/refuse care – assumed
• Multi agency effectiveness – lack of multi agency understanding and sharing of risk and policies followed accordingly.
• Exploration of the role of partner/carer and failure to provide/ask for care

Scope of the review
Areas to be considered as part of the review:

• Whether services met expected professional standards
• Whether there is evidence of professional curiosity when assuming capacity
• Whether safeguarding and other policies and procedures appear to have been shared
• Whether agency communication was effective
• Whether there are examples of good practice

In addition there has been another referral, Adult R where criteria was not met for SAR, but identification of themes which sit across both referrals to be included in this review:

• Use of risk assessment to explore capacity
• Working with self-neglect – whether individuals are fully informed of the consequences of their decisions to refuse care and support
• Role of family member/spouse as carer – assessment of their needs/willingness/ability to provide the care needed their voice.
• Awareness and use of high risk professional guidance and legal guidance to support decision making and multi agency working in cases like this.

Experience required for the Review

• Experience working at a senior level within Health and/or Adult Social Care, and good safeguarding knowledge
• Experience of undertaking Case or Service Improvement Reviews
• A strong understanding of the Mental Capacity Act
