Barnet Safeguarding Adults Board
The London Borough of Barnet Safeguarding Adult Board is seeking to identify an independent reviewer to complete a newly commissioned Safeguarding Adult Review.
Case Overview
The review relates to a woman who was placed in a care home to manage her diabetes but with no other care and support needs. There were however concerns about her daughter who has mental health difficulties interfering with her diabetes management. The lady had capacity and wished to move back home to live with her daughter and was supported to do so.
As part of supporting her to return home professionals from adult social care the district nursing service (the Central London Community Healthcare NHS Trust nursing service) and mental health (North London NHS Foundation Trust) were all invited to work together to ensure safe transition back to her home.
The woman was found deceased at home shortly after returning.

Draw on the relevant experience you have from your background, the soft skills you can bring to this review & the vision you have for the review in question. A well crafted application with reference to the circumstances of the particular review is more relevant and desirable than a standard application designed for any review which references a bank of review experience.
