Dudley Safeguarding People Partnership
Dudley Safeguarding People Partnership are looking to commission a SAR Chair/Reviewer.
The review relates to a woman of African descent who was found deceased in her home.
She came to the UK in 1997, and it is known that she had mental health issues linked to her
experiences from trauma associated to sexual abuse in Africa prior to being trafficked to the
UK.
This adult was a victim of domestic abuse from partners, had a son that was removed from
her care and this we know caused her distress and increased her use of substances, and
repeated phone calls to police threatening to take her own life.
Review themes
Key areas of focus are:
- Agencies understanding of FK complex trauma from being trafficked and
history of sexual abuse
- How agencies supported her when life events triggered her drinking
- Aspects of unconscious bias cultural awareness and competence.
- Information sharing – known to multiple agencies however information was
not coordinated, and no agency had a true understanding of her life story and
the impact of the trauma she experienced.
- Mental Health support offered to FK
- Professional curiosity and use of the escalation and resolution procedures
- Multiple safeguarding concerns raised that led to NFA.
