Dudley Safeguarding People Partnership
The Dudley Safeguarding Adult Board is seeking to identify an experienced Independent Reviewer to complete a Safeguarding Adult Review (SAR) to commence in January 2026.
Case Overview
This Safeguarding Adult Review (SAR) concerns RT, a 49-year-old female with paraplegia resulting from complications following a drug overdose. RT had a history of substance misuse and was paralysed from the neck down. She resided with her partner, who was believed to be actively involved in her drug use.
On the morning of 8 July 2025, RT was found unresponsive at her partner’s home following a night of drug use. Tragically, she passed away, and her partner was subsequently arrested on suspicion of manslaughter.

A well crafted application with reference to the circumstances of the particular review is more relevant and desirable than a standard application designed for any review which references a bank of review experience.
