Gloucestershire Safeguarding Adults Board
The Gloucestershire Safeguarding Adults Board is seeking an independent reviewer to complete a report on a newly commissioned Safeguarding Adult Review (SAR).
Case Overview
The review relates to a woman with care and support needs who was 87. VC died in November 2023; she had dementia and could be verbally and physically aggressive towards her husband. There were repeat concerns about neglect by VC’s husband, as her carer, and there was long standing domestic abuse (husband to wife).
Domestic abuse was noted by several agencies, but it was only VC’s daughter that made safeguarding referrals. There was no DASH completed. There was little professional curiosity recorded about the domestic abuse reported to agencies by the daughter from the husband. There was also no request for a carers assessment.
VC had been on the floor overnight, found covered in urine and faeces with bruises to her face, eye and groin, she had also sustained a head injury. Husband stated he was trying to get her to bed on the stairlift, but she was refusing and she fell, he then dragged her in to the lounge and left her there until the morning when her carers visited and found her. VC was taken to hospital, a CT scan showed a large subdural haemorrhage, she later died. VC had older scratches and bruises on her body as well.
The SAR sub group felt that there was learning as a result of VC's care and support needs and wanted to take it forward as a SAR rather than a DHR/DARDR. The Home Office have agreed that a SAR can be conducted instead of a DHR/DARDR (not joint review), as long as the domestic abuse element is included within the Terms of Reference and the SAR review.
As well as domestic abuse and professional curiosity, we would like to explore whether VC’s dementia stopped professionals from making referrals for safeguarding or DASH etc (perhaps thinking it was carer burn out?).
