Hammersmith and Fulham Safeguarding Adult Board
Hammersmith & Fulham Safeguarding Adult Board (H&F SAB) are looking to commission a SAR Chair/Reviewer.
This Safeguarding Adult Review has been commissioned in response to case of Adult I, a 54-year-old male with a history multiple and complex health and substance use. Adult I required support with mobility, and carers provided support with daily living tasks. Adult I also had a history of rough sleeping dating back to his childhood and experienced neglect in his childhood.
The H&F Safeguarding Adults Case Review Group have agreed that this review will seek to identify the
lessons learned from Adult I's case and provide recommendations to prevent future harm. As part of this
objective, this review will also consider the impact of actions taken in response to the recommendations
from SAR Brian, published in 2023 and identify recurring themes. Key lines of enquiry will include:
▪ Reviewing the impact of SAR Brian, considering local responses to previous recommendations:
what has been changed or actioned?
▪ Identifying recurring learning in areas such as:
o Impact of homelessness, past trauma, and drug use on engagement
o Effectiveness of information sharing, case coordination and decision making.
o Understanding of organisational duties, including exploration of practical aspects of
care and support, and implementation of multi-agency plans to manage risk.
▪ Identifying any new areas of learning, including exploration of the role played by palliative care
in order to understand any positive impacts of this service and identify any opportunities for closer integration or different approaches during end-of-life care.
The independent reviewer will be expected to lead on engagement with frontline practitioners and managers to enable reflection on challenges experienced in working with Adult I and enhance the partnerships understanding of barriers to implementing learning from SAR Brian.
