 Hammersmith and Fulham Safeguarding Adult Board

 

Hammersmith & Fulham Safeguarding Adult Board (H&F SAB) are looking to commission a SAR Chair/Reviewer.

 

 

This Safeguarding Adult Review has been commissioned in response to the death of Adult J in February 2025 following an assault in her home.

Adult J was a White British woman in her mid-30s who required support in relation to her mental health and substance use, with a number of different agencies engaged in providing support to Adult J.


Previous referrals to services indicate that Adult J had experienced cuckooing, and financial, physical and psychological abuse with historic records indicating domestic abuse.


The Safeguarding Adults Case Review Group have agreed that the following areas should be considered as part of the review:

• Coercion and control and impact on decision making

o What was understood by services about Adult J’s recognition of risk of abuse, exploitation and cuckooing?

o Was there sufficient understanding of the impact of coercion and control on decision making capabilities?

• Multi-agency frameworks and interventions

o Does this case allow us to draw out wider learning in respect of multi-agency processes for assessing risk, protection planning, and co-ordination of action, including utilisation of Section 42 frameworks?

o How effective were established multi-agency panels such as the local Cuckooing Panel and Community Multi-Agency Risk Assessment Conference (CMARAC) in assisting the management of risk?
