 Hammersmith and Fulham Safeguarding Adults Board 

Hammersmith and Fulham are seeking an independent reviewer for a SAR.

This Safeguarding Adult Review has been commissioned in response to case of Adult H, a 22-year-old care leaver who experienced poor mental health which was compounded by high-risk drug use. Adult H also engaged in high-risk sexual meetings which would involve chemsex and had previously made allegations of sexual assault and rape. He would often decline or disengage from support services and had previously attempted suicide in August 2024. During a welfare check by hostel staff in December 2024, Adult H was found to be unresponsive.

Adult H is one of four recent chemsex deaths in LBHF since September 2024. As this is a new and emerging phenomenon noticed by NHS, drug treatment and criminal justice agencies, we want to understand thematically what can be learned from these four deaths in addition to the specific learning from Adult H’s case.


The Safeguarding Adults Case Review Group and SAR Panel have agreed that the following themes should be considered as part of the Review:

• Thematic analysis inc. protected characteristics and intersectionality

• Identifying lessons to improve identification, early intervention, and multi-agency responses

• Exploring the role of social media and digital platforms and potential safeguarding implications

• Exploring how presenting risks are recognised and responded to in line with local safeguarding policies, clinical pathways and strategic priorities for community safety and harm reduction

• Transitional safeguarding, exploring how effectively services respond to the needs of young adults, including care experienced young people and adults
