Lancashire Safeguarding Adult Board Safeguarding Adult Review & Domestic Homicide Review
The Lancashire Safeguarding Adult Board, together with Pennine Community Safety Partnership is seeking to identify an independent Chair/Author to complete a newly commissioned joint Domestic Homicide Review and Safeguarding Adult Review between February 2026 to October 2026.
Case Overview
The review relates to a young female with care and support needs in her 20s. Their adverse childhood experiences, drug and alcohol use, pain management and domestic abuse impacted on her mental health and suicidal intent. A young person transitioning from care to living independently, she had long-standing mental health difficulties combined with substance misuse.
Factors in this case include the above, including the transition of services for young people to adult services, the associated pain of ketamine use and the effectiveness of multi-agency working.
