North Yorkshire Safeguarding Adults Board

 

The North Yorkshire Safeguarding Adult Board is seeking to identify an independent reviewer to complete a newly commissioned Safeguarding Adult Review (SAR).

 

Case Overview

‘Nikita’ was known to HAS (16 safeguarding referrals) and local NHS FT with multiple attendances at A&E across 2023 and 2024 for suicidal ideation, intentional overdose, and drug/alcohol intoxication. She had a diagnosis of autism and bipolar disorder. On 10/01/24 she attended A&E reporting that her ex-partner had pushed her down the stairs and strangled her to near unconsciousness. A safeguarding referral was made to NYC, but the local NHS FT safeguarding team were not notified. There is no evidence of a DASH risk assessment, referral to MARAC or police involvement at the time. Temporary accommodation was sourced for her.  Nikita continued to present to A&E, but she often self-discharged. Safeguarding concerns were raised again to NYC (26/4/24), but again the HDFT safeguarding team were not included in the referral. It is noted that Nikita also reported a sexual assault to the police in January 2024, but she denied sexual assault when she presented in A&E (10/1/24). 

There appears to be evidence of silo working, limited follow-up, and safeguarding referrals potentially being closed due to non-engagement along with possible failures to escalate domestic abuse risks, leading to missed opportunities for coordinated intervention.

The SAR subgroup members agreed that the criteria for a mandatory SAR under s44 were met due to:

  • Nikita had support significant care and support needs relating to mental health, substance misuse, domestic abuse and exploitation, housing instability, non-engagement with services.
  • Lack of coordinated multiagency working around Mental Capacity, Safety Planning and Information Sharing.
  • Nikita died as a result of drug and alcohol use as a form of self-neglect.

  • Chronic homelessness causing destabilisation.
  • Domestic abuse.
  • Multifaceted care needs because of her lived experience and learning needs
  • The impact of transition to adulthood.
  • Coordinated safety planning.
  • Application of the Mental Capacity Act principles.
  • Barriers to multi-agency working & information sharing.
