Nottinghamshire Children’s Safeguarding Partnership seeks to commission an LCSPR author reviewer as detailed below.
This review concerns a young child who was admitted to hospital with serious injuries that medical professionals concluded were non-accidental and had occurred over a prolonged period. The child presented with a deep rectal tear, multiple fractures, and signs of older injuries. These findings led clinicians to raise immediate safeguarding concerns regarding both physical and sexual abuse. Clinical opinion is that the timing and pattern of the injuries suggest repeated harm, with the potential for significant long-term emotional and psychological impact.
Prior to this incident, agencies had documented concerns regarding neglect, inconsistent engagement with services, and the child’s exposure to individuals and environments that posed a safeguarding risk. Previous referrals highlighted issues such as periods of homelessness, irregular medical attendance, and domestic abuse within the household.
In commissioning this review, the partnership is mindful of recent cases in Nottinghamshire involving neglect and sexual abuse, which have raised similar themes around multi-agency response, risk identification, and the safeguarding of highly vulnerable children. This LCSPR will therefore include a thematic consideration of these patterns to understand whether there are emerging system issues that require attention beyond the specifics of this single case.
