Joint Safeguarding Adult Review and DARDR for the Cumbria Safeguarding Adults Board


Cumbria Safeguarding Adults Board 
is seeking to commission a reviewer for a Joint Safeguarding Adult Review and DARDR

 

Case Overview

The individual was aged 33 at the time of her death and had a history of using illegal drugs but was receiving a managed prescribed drug treatment programme, She was open to probation with the current order dating back to September 2023.

 

There were concerns that the individual was in an abusive relationship which included domestic abuse and coercion and control by both her partner and potentially her partners father which whom she lived. 

 

It is understood that her partner was a registered carer with concerns he restricted her access to the drug treatment programme and other appointments for physical health concerns.  In addition there were concerns in relation to financial abuse and not allowing access to money resulting in a lack of ability to maintain her health and wellbeing.

 

Consent was given for referral to emergency accommodation to protect the individual however this failed to keep her safe and she returned back to living with her partner and his father.

 

It is understood the death has been referred to the Coroner and potentially will be referred to Drug Deaths panel.

 

The individual lived in Cumbria at the time of her death, had care and support needs, and there was concern about how agencies worked together to protect her from the abuse and neglect.
