Hammersmith & Fulham Safeguarding Adult Board (H&F SAB) are looking to commission an independent reviewer for a thematic SAR.

This Safeguarding Adults Review has been commissioned in response to two separate referrals reviewed by the H&F Safeguarding Adults Case Review Group where domestic abuse was a feature.
In both cases there was identification of an adult risk who had experienced significant harm as a result of the abuse, and it was agreed that Section 44 criteria for a Safeguarding Adults Review was met.
Case E
Adult E and Adult A are two sisters who lived together in a privately rented flat. Adult E acted as Adult A’s main carer (supporting her in managing mental health needs), but Adult E herself had her own care and support needs relating to physical and mental health, and the chronology shows that Adult A would often take on the caring role. In March 2024 Adult A stabbed Adult E and was subsequently arrested for attempted murder.
Case F
Adult F is a woman in her 90s with a diagnosis of dementia who lived with her husband, Adult B, a man in his 80s who acted as Adult F’s main carer and had his own care and support needs. In May 2024, Adult F was attacked by Adult B who stabbed her in the abdomen with a kitchen knife and attempted to strangle her.
Whilst there were different circumstances in the two cases, it was felt that there were similar themes, and it was proposed that a joint approach could be taken to the reviews.
The Safeguarding Adults Case Review Group and SAR Panel have agreed that the following themes should be considered as part of the Review, with questions provided as prompts for further exploration:
• Identification and response to domestic abuse, including exploration of generational and cultural silence
• Multi-agency responses
• Support for carers
• Application of the Mental Capacity Act
