A Safeguarding Adults Review for Herefordshire Safeguarding Adults Board
The Herefordshire Safeguarding Adults Board is seeking an Independent Author for a joint review into the suicide of a mother and suspected suicide of her daughter.
The circumstances are of a mother who had instances of poor mental health who cared for her 43 year old daughter who had a LD diagnosis and also suffered with poor mental health. The daughter had recently returned to live at the family home after a period of living in care. Some concerns had been raised previously with regard to the volatility of the relationship. There were no other family members.
