Safeguarding Adult Review for the Hertfordshire Safeguarding  Partnership Board


Hertfordshire Safeguarding Adult Board 
is seeking to commission a reviewer for a Safeguarding Adult Review.

 

The review relates to the death of a middle-aged male. The HSAB is electing to undertake a SAR as they want to explore lessons for single and multi-agency working. The male was employed as part of the emergency services and had been in the military. He had a history of PTSD, alcohol misuse and mental health problems and at times struggled to meet his own care needs. There is evidence of good multi and single agency working but there is a need to explore whether there was consistent and appropriate multi-agency information sharing and intervention.

Review themes

Key areas of focus are being developed but include:

  • Was there effective multi-agency response to the male’s issues including his PTSD, alcohol misuse, mental health and self-neglect?
  • This would include reviewing the relationship between the support offered through employment and wider agencies, 

 

We will have to confirm with the reviewer at the point of commissioning what the precise remuneration levels will be for this review.
