SAR for the Lancashire Safeguarding Adults Board
The Lancashire Safeguarding Adults Board is looking for a lead reviewer to complete a Safeguarding Adults Review (SAR).
The review relates to a young man (late 20s) with care and support needs, and a diagnosis of Autism and Asperger's who sadly died in May 2024. A SAR is being commissioned due to concerns of neglect and acts of omission, with multiple agencies involved in his care prior to his death.
Concerns include his interaction and contact with his care agency, efficacy of safeguarding referrals, and discharge planning, record keeping and decision making re capacity assessment. Substance abuse of alcohol and drugs impacted his care and support needs and domestic abuse was a factor as well.
Review themes
Key areas of focus are:
- Oversight of physical care needs, including substance misuse, and deteriorating presentation
- Service provision for those who are neurodiverse
- Mental Capacity Assessments and Executive Function in relation to Substance Misuse
- Multi-agency working.

- a statement that you do not have any current or historical connection to the agencies/organisations/individuals involved in the review
- confirmation of your availability from May 2025 to November 2025.

A well crafted application with reference to the circumstances of the particular review is more relevant and desirable than a standard application designed for any review which references a bank of review experience.
