Two Review Opportunities

Two SAR's for the Manchester Safeguarding Partnership

Manchester Safeguarding Partnership is seeking to commission independent reviewers for the following two Safeguarding Adult Reviews:



Summary of the Case A:

Case A relates to an 18-year-old care leaver who was living in temporary accommodation when they tragically died. Amongst other concerns, it is expected that the SAR will focus in particular on issues relating to transitional safeguarding practice and the appropriateness of adult temporary accommodation for highly vulnerable young adults leaving care. Further information is included in the attached request.

The review will consider themes in relation to:

  •  Delays, ownership, and management of the transitions process.
  •  Impact of delayed transitions on commissioning appropriate and timely support.
  •  The appropriateness of adult temporary accommodation for highly vulnerable young adults leaving care.
  •  Service response to combined diagnosis of autism, ADHD and mental health issues (depression and suicidal ideation).
  •  Challenges engaging and providing support to prepare for adulthood.
  •  Impact of Adverse Childhood Experiences and trauma on decision making as an adult. 
  •  Trauma informed response by agencies.
  •  Perceptions/responses dealing with a young person who has been a victim of abuse who goes on to becomes a risk to others.
  •  Possible self-neglect.

 

 

Summary of the Case B:

Case B is a Thematic Review involving 3 adults aged between 52 and 65 when they tragically died. Amongst other concerns, it is expected that the SAR will focus in particular on the overshadowing of financial abuse and cuckooing in cases involving long term physical health issues, mental health concerns and self-neglect. The review will consider the service response to financial abuse where the person also relies on the family member or informal carer committing the abuse for their care and support. Further information is included in the attached request.

 

The review will consider themes in relation to:

  •  Overshadowing of financial abuse and cuckooing in cases involving physical and mental health concerns.
  •  Service response to financial abuse where the person relies on the abuser for care and support.
  •  Pathways for referring and responding to suspected financial abuse and/or cuckooing.
  •  Previous involvement with the Criminal Justice System and transfer from custody to appropriate accommodation.
  •  Unconscious bias relating to perpetrators of crime who are being exploited by others.
  •  Service response to combined diagnosis of physical health needs, drug and alcohol dependency and mental health issues. 
  •  Mental capacity and non-concordance with medical treatment.
  •  Risk management and role of the lead professional in cases involving complex safeguarding.
  •  Trauma informed response by agencies.
