Two Review Opportunities

Two SAR's for the Stockport Safeguarding Adults Partnership

Stockport Safeguarding Adults Partnership seeks two reviewers for two SAR's.

Summary of the Case 1:

The SAR is in relation to the deaths of four women whilst in hospital.  The deaths were not related and occurred over a 20 month period. The women were not ordinary residents of Stockport and the SAR panel in this case therefore involves representation from other Board areas.

Review themes

Members of the Stockport Safeguarding Adults Partnership have determined that the themes to be covered in this review are:

  •  Information sharing and communication between agencies
  •  How advocacy for individual adults and their families is publicised and understood by multi-agency teams
  •  The handling of allegations against staff, including understanding of PIPOT processes and legislation, and safer recruitment strategies
  •  Commissioning arrangements around out-of-area placements and the responsibilities of placing and host authorities
  •  How all relevant agencies are involved in safeguarding investigations
  •  The multi-agency process for reviewing placements and ensuring placements meet the needs of vulnerable adults

Summary of the Case 2:

The SAR is in relation to a 76-year-old female who died whilst in hospital.  She had mixed Alzheimer’s and vascular dementia, and had a grade 2 pressure ulcer.

At the inquest HM Coroner concluded the adult died from osteomyelitis where the deterioration of the sacral pressure ulcer was not recognised until she became very unwell and attempts to treat it were unsuccessful.

Review themes

Members of the Stockport Safeguarding Adults Partnership have determined that the themes to be covered in this review are:

  •  Information sharing and communication between agencies and the commissioned home care provider
  •  The use of advocacy and how agencies communicate with family members
  •  Multi-agency Section 42 processes and provider-led enquiries

Draw on the relevant experience you have from your background, the soft skills you can bring to this review & the vision you have for the review in question. A well crafted application with reference to the circumstances of the particular review is more relevant and desirable than a standard application designed for any review which references a bank of review experience.
