Stockton-on-Tees seeks an independent reviewer for a Safeguarding Adults Review

The Teeswide Safeguarding Adults Board (TSAB) is a four Local Authority SAB and covers the areas of Hartlepool, Middlesbrough, Stockton-on-Tees and Redcar & Cleveland.

 

This case involves a young woman (referred to as SLBS) aged 19. SLBS had long standing mental health needs and attempted suicide on a few occasions. She was known to the Local Authority Care Experienced Team and received support from mental health and other support services. SLBS was accepting of support. SLBS’s mother was very ill in hospital over several months and SLBS found this particularly difficult to cope with. Her mother then passed away, which further impacted on SLBS’s mental health and ability to navigate and complete practical tasks which needed to be done following her mother’s death. SLBS sadly took her own life, the cause of death is suspected to be due to an overdose – the Coroner is yet to confirm.

 

The focus will be on the following areas:

 

  • Care Experienced individuals and transition
  • Bereavement support and young people coping with and navigating what needs to be done following a person’s death
  • Housing issues following a person’s death
  • Impact of benefits being suspended
  • Family involvement - how to manage cases where the individual does not consent to wider family being made aware of circumstances (for example being a child in care). Understanding family dynamics and family safety planning.
