 Telford and Wrekin Safeguarding Partnership is seeking an independent reviewer for a Thematic Safeguarding Adults Review

 

The thematic Safeguarding Adults Review will cover the following cases:

Adult P - Death of a 48 year old male who struggled to mobilise and required carer support from his mother and his friend. Male had multiple health concerns including visual impairment, above knee amputation, Diabetes – poorly managed, Depression/Anxiety. He was admitted to hospital where on arrival he was hypothermic, temp 26c. The initial working diagnosis was possible sepsis secondary to toe wound and ketoacidosis secondary to prolonged starvation. Cause of death was:

1a. Sepsis

1b. Pneumonia

  1. Diabetes Mellitus

 

Adult J – Death of a 56 year old male who had care and support needs in respect of pressure ulcers and pressure area care. Past Medical History included: Falls, Multiple episodes of Diabetic Ketone Acidosis ( DKA’s), Alcohol Excess, Type 1 diabetes mellitus, Dialysis ESRF x 3 weekly, previous IV Drug Abuse and Pleural effusion. Male was well known to services and had a history of being abusive to staff members which would cause care packages to break down. The management of his diabetes would impact on his levels of aggression and capacity. He was admitted to hospital following a fall and sadly died of:

A End stage renal disease

1.b Diabetic Neuropathy

1.C Type 1 diabetes

2. Ischaemic heart disease and peripheral vascular disease.

 

 

Review Themes

The thematic review will examine the following issue(s) which are themes across both SAR’s:

 

  • Self-neglect and impact of substance misuse
  • Aggression to professionals and impact on care provided
  • Management of long term health conditions
  • The need for difficult conversations to be had between individuals and care givers concerning living circumstances and medical conditions
  • Capacity

 

Reviewers should demonstrate:

  • An understanding of safeguarding adults at risk.
  • Ability to chair and present statutory reports to multi-agency meetings.
  • Experience and/or training in case reviews
  • Experience of facilitation of Practitioner Learning Events 
  • An understanding of different review methodologies and working to the SCIE SAR Quality Markers. 
  • Knowledge of relevant legislation and issues relating to mental health, learning disabilities and safeguarding.
