Review Opportunity

SAR for the City of York Safeguarding Adults Board


The City of York Safeguarding Adults Board 
is commissioning a reviewer for a SAR. This review did begin with a previous author, so some of the work has been completed. A proportionate SILP model will be applied to completing this piece of work. 



Case:

This SAR relates to a 94yr man living with dementia, who lived alone in York, with support from a care provider, and family who lived close by. He was admitted to hospital following a fall at home, though was soon well enough to return home. However, his discharge arrangements were ceased due to safeguarding issues being raised.  Whilst in hospital he experienced a significant number of moves, and further falls, and sadly became ‘end of life’ following a third fall, and he died in hospital.

Significant concerns were raised by his family about his care, treatment and communication between agencies. 

Review themes

The review will consider themes relating to:

  •  Information sharing and communication between agencies and family members.
  •  Hospital discharge arrangements. 
  •  Mental Capacity Act and Deprivation of Liberty Safeguards practice.
  •  Advocacy arrangements and responses.
  •  Understanding of Safeguarding Adults principles and processes .

Experience required:

The Independent Reviewer will be required to evidence the following: 

  • A highly developed knowledge and experience of safeguarding adults at risk.
  •  Experience of chairing and presenting to multi-agency meetings. 
  • Knowledge of relevant legislation and issues relating to safeguarding, MCA and DoLS.
