Rotherham Safeguarding Adults
The Rotherham Safeguarding Adults Board is seeking to identify an experienced Independent Reviewer to complete a Safeguarding Adult Review (SAR) to commence in January 2026.
Case Overview
Adult B was 39 years old when he passed away in the hospital. B died in July 2024. The coroner is yet to confirm a cause of death.
Medical Needs
B had diabetes, which was poorly managed, and is reported to regularly become ‘severely unwell with complications from chronic health conditions.’
B self-reported having issues with his bowel, which resulted in incontinence, although the cause of this was unknown, and he had anxiety and depression. Towards the end of his life, there was an escalation in his mental health, including overdoses and non-compliance with medication, with the intent to harm himself.
Care Act and Mental Capacity Assessments
Between February 2023 and B's death in July 2024, there were 8 professional referrals to request a care act assessment. During this time, B was spoken to 3 times by adult social care (ASC) – each time, he reported multiple concerns and said that he needed support. On one occasion, he was deemed to have capacity by ASC and was deemed to be choosing not to take his insulin- there is no evidence of a capacity assessment being completed. On a further occasion, he was deemed as not meeting the criteria for a care act assessment, despite the 5 previous referrals evidencing eligibility and conversations with B where he said he needed daily support. There is no evidence of how or why B was considered ineligible for completion of a care act assessment (CAA).
No care act assessment was ever completed for B.
Professional referrals also raised significant safeguarding concerns in relation to self-neglect.
Homelessness
In February 2023, B left his accommodation due to being targeted by other residents and presented as homeless. He reported that he was also struggling to abstain from substances at his accommodation due to most other residents using drugs. He said he didn't feel safe, so he chose to hand in his keys. The homeless team was of the view that B wasn't technically homeless, so they would not provide him with temporary accommodation. Housing advised that he needed to report his concerns to the police before he would be offered temporary accommodation. B did not wish to speak to the police as he was fearful of repercussions. This resulted in B sleeping rough.
