Staffordshire and Stoke-on-Trent Safeguarding Adult Partnership Board are looking to commission independent reviewers for 4 safeguarding adult reviews.
Case 1: An adult with learning difficulties who experienced neglect from family members he lived with. They were admitted to hospital with malnutrition, some physical injuries and pneumonia and passed away during the time in hospital.
Case 2: Historical decline in wellbeing and the subsequent self-neglect resulted in placement under a Deprivation of Liberty Safeguards (DoLS). Adult A objected to this placement and made allegations of rape against care staff. The initial closure of the safeguarding concern by Adult Social Care and the police, despite allegations and subsequent fear, suggests there might have been gaps in the response to their needs and that the adults voice was not heard. The fact that Adult A was assessed as needing a high level of support (1:1) but was not seen or spoken to for 19 hours before their death indicates potential neglect or failure in care provision.
Case 3: This case involves a married couple, with husband having a caring role for his wife who had a cognitive impairment. Both had care and support needs, and caring capacity was not considered as these escalated and at the point of diagnosis of a terminal illness. There are issues relating to the sharing of information and assessment of risk following family concerns about low mood and possession of firearms, including as part of discharge planning. This case unfortunately resulted in the death of both husband and wife.
Case 4: This case involves a vulnerable adult who experienced significant trauma in childhood and as an adult. This includes physical abuse, domestic violence, bereavement and sexual abuse. The adult was known to mental health and drug and alcohol services, with varied levels of engagement. She was also assessed as having care and support needs and was known to police and other services as experiencing domestic abuse in relationships. The adult was present and arrested following the death of an adult male.
