This review follows a tragic incident involving the deaths of two young children and concerns regarding their siblings and parents. The case includes complex multi-agency involvement, maternal mental health, cross-border medical history, and significant issues relating to equality, equity, diversity and inclusion (EEDI).

The LCSPR will examine:

  • Understanding of mental health needs, risk and safety planning.
  • Responses to historic, international and UK medical information.

Cross-Border Considerations

  • Challenges interpreting medical history and treatment received outside the UK.

Language & Communication

  • Use of interpreters.
  • Reliance on family members for translation.

Equality, Equity, Diversity & Inclusion

  • Cultural identity, faith and community context.

Information Sharing & Collaboration

  • Triangulation of concerns and early years inclusion.

Voice of the Child

  • Understanding children’s lived experience, particularly very young or non-verbal children.
