Accredited SILP™ Reviewers who join Momentum receive opportunities to apply through Review Consulting to undertake SILP reviews.

Staffordshire and Stoke-on-Trent Safeguarding Adult Board

Staffordshire and Stoke-on-Trent Safeguarding Adult Board is looking to commission reviewers for 4 Safeguarding Adult Reviews. 

  

Case 1: A case involving self-neglect and severe hoarding that contributed to an adult’s death. Significant factors include alcohol use and impaired executive function, with an exploration of whether past professional relationships influenced perceptions or responses to risk. Knowledge of understanding around the mental health conditions associated with hoarding would be beneficial but not essential.

 

Case 2: A case exploring missed opportunities to engage, demonstrate professional curiosity, and support an adult showing signs of self-neglect, as well as her son, who assumed an informal caring role.

 

Case 3: This case involves an adult with severe autism, whose death was as a result of neglect where the family failed to seek medical attention. There is also potential learning around understanding and information sharing in the child in need processes/transition to adult services.

 

Case 4: Neglect of an adult with a severe learning disability and other health conditions, who died shortly following a move from the family home to a residential setting. Areas of concern include appropriateness of placements, medication management and how this impacted behaviours and coordination of care. A LeDeR review and Prevention of Future Deaths report will also inform the review.

A well crafted application with reference to the circumstances of the particular review is more relevant and desirable than a standard application designed for any review which references a bank of review experience.
